SCOHR

System for California Oral Health Reporting (SCOHR) was developed by the SJCOE (CEDR Department) and in collaboration the ACSA, CDA, CDE, and CCSESA, to provide an online and centralized method of collecting and centralizing all required student oral health assessment data pertaining to Assembly Bill 1433.

Participating schools and/or districts will have access to input, manage, and track electronic Oral Assessment Request Forms and all oral health assessment data, bulk upload data to pre-fill the Oral Assessment Request Forms, print pre-filled Oral Assessment Request Forms (individually or in-bulk), and export all data into a standardized format.

SCOHR tracks the status of all Oral Assessment Request Forms (when it was generated and/or sent, when it was returned signed, is it pending, signed, etc.). SCOHR also includes Report Generator utility to run ad-hoc, custom lists and aggregate reports, at the district, county, and/or state levels.
